HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
At Sacramento RedLight Therapy, we understand that your health information is personal. We are committed to protecting your protected health information, also called PHI, and following applicable federal privacy laws.
This Notice explains how we may use and disclose your health information, your rights regarding that information, and our legal duties to protect it.
Your Rights
You have the right to inspect and request a copy of your health information, in paper or electronic form, subject to limited exceptions allowed by law. You also have the right to ask us to correct health information you believe is incorrect or incomplete.
You have the right to request confidential communications. For example, you may ask us to contact you at a certain phone number, mailing address, or email address. You may also ask us to limit certain uses or disclosures of your health information. We will consider your request, but the law does not always require us to agree.
You have the right to receive an accounting of certain disclosures of your health information. You also have the right to receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.
If you believe your privacy rights have been violated, you have the right to file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Our Uses and Disclosures
We may use and disclose your health information for treatment, payment, and health care operations. This means we may use your information to provide services, coordinate care, maintain records, improve our services, and, where applicable, obtain payment for services provided. HIPAA permits these core uses and disclosures without separate written authorization in many situations.
We may also use or disclose your health information when required by law, for public health purposes, for health oversight activities, in response to certain legal requests, and for other purposes permitted or required by HIPAA and other applicable laws.
We may share relevant information with family members, close friends, or others involved in your care or payment for your care when appropriate and allowed by law, unless you tell us otherwise.
We will not use or disclose your health information for uses that require your written authorization unless you have given that authorization. If you do authorize us, you may revoke that authorization in writing at any time, subject to limited exceptions. HHS’s model notice also requires providers to address authorization for certain marketing-related uses and disclosures.
Our Responsibilities
We are required by law to maintain the privacy and security of your protected health information, provide you with this Notice, and follow the duties and privacy practices described in this Notice. If a breach occurs that may have compromised the privacy or security of your information, we will notify you as required by law.
We reserve the right to change the terms of this Notice, and any updated Notice will apply to all health information we maintain. If we make material changes, the updated Notice will be made available in our office and on our website. HHS says covered providers must revise the notice when there is a material change and must post it on their website if they maintain one.
Website Posting and Availability
This Notice is available on our website and may also be requested in paper form at any time. Covered health care providers with a direct treatment relationship must provide the notice no later than the date of first service delivery and make a good faith effort to obtain written acknowledgment of receipt.
